Background and indications

Image-guided musculoskeletal injections involve delivering medication (corticosteroids, hyaluronic acid, local anesthetic) directly into a joint, bursa, tendon sheath, or near a nerve root. CT, fluoroscopic, or ultrasound guidance achieves needle placement accuracy above 95%, compared to 60-80% for blind injections, improving efficacy and reducing complications.

Main indications include: symptomatic osteoarthritis (knee, hip, shoulder, facet joints), radiculopathy (disc herniation, foraminal stenosis), tendinopathy, bursitis, and adhesive capsulitis.

Pre-procedure assessment

Assessment includes recent imaging (radiographs, MRI, or CT) of the target area, coagulation panel (PT, aPTT, platelets), and verification that anticoagulant therapy has been appropriately managed according to guidelines. Allergy history (contrast agents, anesthetics) is verified.

Procedure

After rigorous skin disinfection, the needle is positioned under imaging guidance (CT, fluoroscopy, or ultrasound depending on the anatomical target). A test injection of contrast may confirm proper positioning. The medication is then injected slowly. The procedure takes 10 to 20 minutes. Spinal injections (epidural, foraminal, facet) are preferentially performed under fluoroscopic or CT guidance.

Results and scientific evidence

Image-guided injections have demonstrated significantly superior needle placement accuracy compared to blind injections. For foraminal epidural injections, studies showed 50-75% pain relief at 3 months for disc herniation radiculopathy (Benny et al., The Spine Journal, 2011; DOI: 10.1016/j.spinee.2011.04.004). For knee osteoarthritis, image-guided corticosteroid injections provide 4 to 12 weeks of relief.

Hyaluronic acid (viscosupplementation) may extend the effect to 6-12 months in moderate osteoarthritis.

Risks and complications

Complications are rare thanks to imaging guidance: infection (< 0.1%), puncture site bleeding (< 1%), vasovagal reaction (< 2%), transient facial flushing after corticosteroids (5-10%, benign). Spinal injections carry a rare risk of post-dural puncture headache (< 1%) and exceptional risk of neurological complications.

Recovery

The patient can resume normal activities the same day. Corticosteroid effect manifests within 24-72 hours. Transient pain increase in the first hours is possible (product reaction). Avoiding intense efforts for 48 hours is recommended.

Practical information

The procedure is performed on an outpatient basis. It is performed by an interventional radiologist.
